Direct Support Professional 

Upreach, LLC has been dedicated to providing residential-based services to individuals with physical and developmental disabilities since 2003. Based in Columbus, our mission at the Upreach family is to build foundations for optimal living for our beloved consumers.

What Does a Direct Support Professional Do?

Direct Support Professionals are the most important part of the Upreach team, and will spend most of their time with our consumers. We are looking for candidates with a passion for helping and making positive impacts in the lives of others - a Direct Support Professional is expected to provide a high quality of care in our individuals' homes.

Direct Support Professional Responsibilities:

  • Provide supervision and guidance to each consumer in accordance with his/her Individual Service Plan.

  • Assist each consumer in planning and implementing in-home and community-based activities.

  • Administer medications where indicated and appropriate.

  • Act as role models and promote positive lifestyles for each consumer.

  • Advocate for the safety and well-being of each consumer to appropriate management personnel.

  • Provide physical assistance as needed, including but not limited to, personal care, housekeeping and transportation

Direct Support Professional Requirements:

  • High School Diploma or equivalent.

  • Current and valid Ohio Driver License with no more than 6 points on driving record.

  • Working and safe motor vehicle, maintaining minimum of State required insurance coverage

  • No felony convictions as identified and outlined by the Ohio Revised Code.

Job Types: Full-time, Part-time

Salary: $12.00 - 13.00 /hour
